Risk factors for carbapenemase-producing organisms among inpatients in Scotland: A national matched case–control study

Objective:
To determine risk factors for carbapenemase-producing organisms (CPOs) and to determine the prognostic impact of CPOs.
Design:A retrospective matched case–control study.
Patients:Inpatients across Scotland in 2010–2016 were included. Patients with a CPO were matched with 2 control groups by hospital, admission date, specimen type, and bacteria. One group comprised patients either infected or colonized with a non-CPO and the other group were general inpatients.
Methods:Conditional logistic regression models were used to identify risk factors for CPO infection and colonization, respectively. Mortality rates and length of postisolation hospitalization were compared between CPO and non-CPO patients.
Results:In total, 70 CPO infection cases (with 210 general inpatient controls and 121 non-CPO controls) and 34 CPO colonization cases (with 102 general inpatient controls and 60 non-CPO controls) were identified. Risk factors for CPO infection versus general inpatients were prior hospital stay (adjusted odds ratio [aOR], 4.05; 95% confidence interval [CI], 1.52–10.78; P = .005), longer hospitalization (aOR, 1.07; 95% CI, 1.04–1.10; P < .001), longer intensive care unit (ICU) stay (aOR, 1.41; 95% CI, 1.01–1.98; P = .045), and immunodeficiency (aOR, 3.68; 95% CI, 1.16–11.66; P = .027). Risk factors for CPO colonization were prior high-dependency unit (HDU) stay (aOR, 11.46; 95% CI, 1.27–103.09; P = .030) and endocrine, nutritional, and metabolic (ENM) diseases (aOR, 3.41; 95% CI, 1.02–11.33; P = .046). Risk factors for CPO infection versus non-CPO infection were prolonged hospitalization (aOR, 1.02; 95% CI, 1.00–1.03; P = .038) and HDU stay (aOR, 1.13; 95% CI, 1.02–1.26; P = .024). No differences in mortality rates were detected between CPO and non-CPO patients. CPO infection was associated with longer hospital stay than non-CPO infection (P = .041).
Conclusions:A history of (prolonged) hospitalization, prolonged ICU or HDU stay; ENM diseases; and being immunocompromised increased risk for CPO. CPO infection was not associated with increased mortality but was associated with prolonged hospital stay.
Weed seed bank emergence across the Corn Belt

Field experiments, conducted from 1991 to 1994, generated information on weed seedbank emergence for 22 site-years from Ohio to Colorado and Minnesota to Missouri. Early spring seedbank densities were estimated through direct extraction of viable seeds from soil cores. Emerged seedlings were recorded periodically, as were daily values for air and soil temperature, and precipitation. Percentages of weed seedbanks that emerged as seedlings were calculated from seedbank and seedling data for each species, and relationships between seedbank emergence and microclimatic variables were sought. Fifteen species were found in 3 or more site-years. Average emergence percentages (and coefficients of variation) of these species were as follows: giant foxtail, 31.2 (84%); velvetleaf, 28.2 (66); kochia, 25.7 (79); Pennsylvania smartweed, 25.1 (65); common purslane, 15.4 (135); common ragweed, 15.0 (110); green foxtail, 8.5 (72); wild proso millet, 6.6 (104); hairy nightshade, 5.2 (62); common sunflower, 5.0 (26); yellow foxtail, 3.4 (67); pigweed species, 3.3 (103); common lambsquarters, 2.7 (111); wild buckwheat, 2.5 (63), and prostrate knotweed, 0.6 (79). Variation among site-years, for some species, could be attributed to microclimate variables thought to induce secondary dormancy in spring. For example, total seasonal emergence percentage of giant foxtail was related positively to the 1st date at which average daily soil temperature at 5 to 10 cm soil depth reached 16 C. Thus, if soil warmed before mid April, secondary dormancy was induced and few seedlings emerged, whereas many seedlings emerged if soil remained cool until June.

Recent Genome-Wide Association Studies (GWAS) have identified four low-penetrance ovarian cancer susceptibility loci. We hypothesized that further moderate- or low-penetrance variants exist among the subset of single-nucleotide polymorphisms (SNPs) not well tagged by the genotyping arrays used in the previous studies, which would account for some of the remaining risk. We therefore conducted a time- and cost-effective stage 1 GWAS on 342 invasive serous cases and 643 controls genotyped on pooled DNA using the high-density Illumina 1M-Duo array. We followed up 20 of the most significantly associated SNPs, which are not well tagged by the lower density arrays used by the published GWAS, and genotyping them on individual DNA. Most of the top 20 SNPs were clearly validated by individually genotyping the samples used in the pools. However, none of the 20 SNPs replicated when tested for association in a much larger stage 2 set of 4,651 cases and 6,966 controls from the Ovarian Cancer Association Consortium. Given that most of the top 20 SNPs from pooling were validated in the same samples by individual genotyping, the lack of replication is likely to be due to the relatively small sample size in our stage 1 GWAS rather than due to problems with the pooling approach. We conclude that there are unlikely to be any moderate or large effects on ovarian cancer risk untagged by less dense arrays. However, our study lacked power to make clear statements on the existence of hitherto untagged small-effect variants.

Summary
Urinalysis is critical in evaluating and treating patients with urinary tract infections or complex urosepsis. Ultrasound guidance for suprapubic bladder catheterization shows improved success rates, decreased number of attempts, and decreased complications. Ultrasound guidance for placement of central venous catheters (CVCs) is one of the most important uses of ultrasound in the clinical setting. Undetected foreign bodies during initial ED visits may lead to complications, including inflammatory reaction, infection, delayed wound healing, poor cosmetic outcome, and, less often, life-threatening illness. Therapeutic thoracentesis is performed when patients are symptomatic due to an effusion. Pneumothorax is the major complication associated with thoracentesis. Lumbar puncture (LP) is one of the most frequently performed procedures in the acute care setting. The amount of peritoneal fluid in patients requiring therapeutic paracentesis may be large enough that ultrasound is not necessary. Unrecognized pericardial tamponade is one of the most quickly fatal pathological entities in medicine.

Summary
As Reingard Nischik notes, Atwood is “one of the most important literary chroniclers of our time” and an international bestseller; thus, the Atwood industry is “booming.” According to Coral Ann Howells, “From The Edible Woman onwards, her novels have focused on contemporary social and political issues,” challenging contemporary social myths and fashionable ideologies and “endlessly surprising her readers with her ongoing experimentalism.” Critics have viewed Atwood's novels using formalist, biographical, psychoanalytic, feminist, Jungian, dialogic, intertextual, phenomenological, narratological, cultural, postmodern, postcolonial, generic, and deconstructionist approaches. Atwood's novels are variously described as realism, romance, ghost story, thriller, memoir, Bildungsroman, Kunstlerroman, science fiction, metafiction, anti-novel, fairy tale, satire, parody, Gothic, dystopian, nationalist, feminist, revisionist, modernist, intramodern, postmodern, and postcolonial.

An interpersonal problem is having a goal or desired interaction involving another person that is not being met. Interpersonal problems are an ongoing part of life. Whether it be deciding where to have lunch with a friend or your neighbour continually parking his car in front of your home, we are involved in interpersonal problems everyday. We are gregarious, and inherent within interaction with others in conflict. As educators we are continually aware of the number of interpersonal difficulties which occur within the classroom everyday.
